If you have a good relationship with a client, is it appropriate to be casual for a dinner meeting?

It depends on the industry. If you are working in the medical industry, for example, it is a very conservative industry. It is also important to remember that the client is the client (and you are 'hired help') at ALL TIMES. Best to error on the side of being to conservative.
